To exchange Rs 2,000 notes in the bank, you will have to show your original ID. This ID can be any one of Aadhaar Card, Driving License, Voter ID Card, Passport, NREGA Card and Population Register.
At the top of this form, you will have to write the name of the bank branch where the notes are being exchanged. After this, if you have a bank account, then enter its number. After this you have to write your name. Photocopy of any one ID card will have to be attached. ID card numbers have to be written. After this, the number and value of 2000 notes will have to be entered in the form. You have to sign on the form. After this the date and place have to be written.
